With SNAP, you can get the supplies you need right on time, delivered directly to your home, without making an extra stop.
How SNAP CPAP Resupply Works
Getting Started – An HME Respiratory team member will set up your account and a SNAP Resupply team member will reach out when you are eligible for your first resupply order.
Easy Reordering – Once set up, you can reorder supplies the way that works best for you:
SNAP CPAP app (Apple App Store or Google Play)
Secure patient portal (Chrome recommended)
Phone call
Email
Real-Time Coverage Check – SNAP verifies your insurance eligibility in real time, so you’ll always know what’s covered and when.
Home Delivery – Orders are shipped directly to your door for added convenience.

SNAP Resupply FAQs
What is the SNAP Patient Portal and how do I access it?
The SNAP Patient Portal is a secure online tool that lets you reorder eligible CPAP supplies, check your order status, update contact or insurance information, and get assistance when needed.
- Access the portal at https://portal.snapworx.com
- You’ll need a valid email address saved in your HME patient account to set up your login
- If you already have an account on the SNAP mobile app, you can use the same login credentials
How do I place a new order?
SNAP will notify you when you’re eligible to place a new order.
From the home screen, select Place a New Order
SNAP will automatically display your eligible items (you can uncheck any you don’t need)
Answer the “Reason for Replacement” for each item, if required
Confirm your shipping address
Click Place My Order Now
If you aren’t yet eligible, SNAP will show how many days remain until your next order.
Tip: The steps are nearly identical whether you’re using the SNAP Patient Portal or the SNAP mobile app.
How do I update my insurance information?
From the home screen, select Insurance Info
View your current insurance details
To update, upload a photo of the front and back of your new insurance card
Save the changes to update your account
What is the SNAP mobile app and how do I access it?
- Place new orders
- Track order status
- Update personal or insurance information
- Get help if needed
The app is available for download from the Apple App Store or Google Play. You’ll use the same login credentials as the Patient Portal.
I'm having issues or need to make changes to my SNAP Resupply account. How can I get help?
HME Home Medical is only able to assist patients who have an active prescription on file with our company.
If you are located in Wisconsin and would like to transfer your prescription to us, please click the green Contact HME button below.
If you are not an HME patient, but need help with your SNAP Resupply ordering or account, please reach out to your current provider. You can find this information on your last packing slip, last invoice, or any delivery documentation you may have. If you do not have this information, you may also reach out to SNAP Resupply directly by emailing SNAPsupport@brightree.com
